Sports Interest Form

All families interested in spring sports should fill out the form below by February 26.

We look forward to everyone’s participation and support. Questions should be addressed to Mrs. Lindsy Patten and Dr. Felix Moreno, our Athletic Directors.

  • Sports forms should be completed and managed on the Appryse website.  

Initial Evaluation - Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, in any school year, the student is required to complete a Comprehensive Initial Pre-Participation Athletic Physical Evaluation form, which must be downloaded from the "Doctor's Physical" section of the home page of your Appryse account and clicking on the button labeled "Download Blank Form".
