Sports Interest Form 2024-2025

All families interested in fall athletics should fill out the Fall Sports 2024-2025 Interest Form. We look forward to everyone’s participation and support.

The Fall Sports Interest form deadline was August 5 and it remained open for two more weeks, but at this point it has been closed due to necessary planning for the upcoming season.

Questions should be addressed to our Athletic Department. 

Sports forms should be completed and managed on the Appryse website.  

Initial Evaluation - Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, in any school year, the student is required to complete a Comprehensive Initial Pre-Participation Athletic Physical Evaluation form, which must be downloaded from the "Doctor's Physical" section of the home page of your Appryse account and clicking on the button labeled "Download Blank Form".
