Sports Interest Form 2024-2025

All families interested in fall athletics should fill out the Winter Sports 2024-2025 Interest Form. We look forward to everyone’s participation and support.

Sports forms should be completed and managed on the Appryse website.  

Initial Evaluation - Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, in any school year, the student is required to complete a Comprehensive Initial Pre-Participation Athletic Physical Evaluation form, which must be downloaded from the "Doctor's Physical" section of the home page of your Appryse account and clicking on the button labeled "Download Blank Form." The form is valid for 1 year and expires after 365 days.
