Sports Interest Form
All families interested in Fall 2026 sports should fill out the form below by July 15.

Initial Evaluation - Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, in any school year, the student is required to complete a Comprehensive Initial Pre-Participation Athletic Physical Evaluation form, which must be downloaded from the "Doctor's Physical" section of the home page of your Appryse account and clicking on the button labeled "Download Blank Form".
