Sports Interest Form 2022-2023

All families interested in spring athletics should fill out the Spring Sports 2023 Interest Form.

Questions should be addressed to Mr. John Schwartz and Mrs. Lindsy Patten. The deadline to complete the Spring Sports Interest Form is February 28.

Sports forms should be completed and managed on the Appryse website.  

Initial Evaluation - Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, in any school year, the student is required to complete a Comprehensive Initial Pre-Participation Athletic Physical Evaluation form, which must be downloaded from the "Doctor's Physical" section of the home page of your Appryse account and clicking on the button labeled "Download Blank Form".
