Sports Interest Form 2023-2024

All families interested in spring athletics should fill out the Fall Sports 2023 Interest Form. The deadline to complete the form is August 4.

Sports forms should be completed and managed on the Appryse website.  

Initial Evaluation - Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, in any school year, the student is required to complete a Comprehensive Initial Pre-Participation Athletic Physical Evaluation form, which must be downloaded from the "Doctor's Physical" section of the home page of your Appryse account and clicking on the button labeled "Download Blank Form".
